CLEVELAND, Ohio — Guardians starting pitcher Ben Lively exited Cleveland’s game against Milwaukee on Monday with an undisclosed injury before throwing a pitch in the fourth inning. Lively stepped to the mound in the top of the fourth and quickly signaled to trainers before being joined by manager Stephen Vogt and pitching coach Carl Willis.
